10/18/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax discussed with QMHPA her morning was well. Intervention: QMHPA began the session by inquiring about Ms. Broadnax current thoughts, feelings, and emotions. QMHPA reinforced the importance of using coping skills when she was experiencing feelings of anxiousness. QMHPA reiterated the use of deep breathing skills, guided imagery, and positive self-talk. QMHPA educated Ms. Broadnax on anxiety and how using coping skills properly can assist with decreasing feelings of anxiousness and stress. QMHPA used modeling and demonstration to educate Ms. Broadnax how to implement strategies consistently. QMHPA reinforced identifying ways that she could work towards reducing/eliminating unwanted stress. QMHPA continued to reinforce the importance of developing coping skills to assist her with overcoming her frequent stress, anxiety and depression such as practicing techniques such as breathing, walking away, listening and understanding what is being said before reacting, calming and relaxing to assist her with controlling/overcoming her stressors. QMHPA encouraged and promoted the importance of improving her communication skills so that she can begin using positive interactions with others and begin developing appropriate decision- making skills. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax identified her mental health as âokayâ. Ms. Broadnax stated that she had taken her medication. Ms. Broadnax was attentive and engaged during review of the importance of being able to identify and utilize community resources. Ms. Broadnax demonstrated the ability to obtain Resource Management Tool. Ms. Broadnax agreed to engage in a community integration activity. During the activity Ms. Broadnax was able to utilize community resources in an effort to identify other resources which may be beneficial in the future. Plan: QMHPA will continue to teach Ms. Broadnax social skills and communication skills through modeling and role-playing to improve her relationships with others 3-4 times (Weekly).
